This Privacy Statement describes how Chehalis Dental Care (“CDC”) collects, uses, and discloses certain personal information obtained through our public web site at chehalisdentalcare.com and affiliated sites. This Privacy Statement does not address information collection through other sources such as phone calls with us, e-mails with us, or in-person encounters.
PERSONAL INFORMATION COLLECTION AND USE

We may share Personal Information among the people in our practice or among people in practices with whom we associate or consult for medical advice.
